Patients with prior COVID-19 at higher risk for new-onset autoimmune inflammatory disease

Risk for autoimmune inflammatory rheumatic disease increased with severity of COVID-19 cases.

Vaccine doses lowered the risk, except among those who had more severe COVID-19.

Patients with a history of COVID-19 demonstrate a higher risk for new-onset autoimmune inflammatory rheumatic diseases, with greater infection severity linked to greater risk, according to data published in the Annals of Internal Medicine.

...Compared with either uninfected or influenza-infected control patients, those infected with SARS-CoV-2 demonstrated a “significantly higher” risk for autoimmune inflammatory rheumatic diseases in both the South Korean and Japanese cohorts, according to the researchers. In the South Korean cohort, risk increased among those with mild (HR = 1.22; 95% CI, 1.15-1.29) and moderate-to-severe (HR = 1.42; 95% CI, 1.27-1.59) cases of COVID-19.

Meanwhile, the risk for autoimmune inflammatory rheumatic disease decreased as patients received additional COVID-19 vaccine doses (one dose HR = 0.59; 95% CI, 0.49-0.71; two or more doses HR = 0.42; 95% CI, 0.34-0.52), regardless of vaccine type. However, this was only true among those who had mild, but not moderate-to-severe, COVID-19, the researchers wrote.

Moderna (NASDAQ:MRNA) announced Tuesday its next-generation COVID-19 vaccine mRNA-1283 reached the primary endpoints in a Phase 3 trial, outperforming the company’s FDA-approved COVID shot mRNA-1273.222.

Unlike the company’s original COVID vaccine, which requires ultra-frozen storage conditions, mRNA-1283 is a refrigeration-stable vaccine formulation that can lead to a longer shelf life and storage benefits.

Citing data from its NextCOVE pivotal trial, Moderna (MRNA) said that mRNA-1283 generated a more robust immune response compared to mRNA-1273.222 against both the Omicron BA.4/BA.5 strains and the original COVID strains.

The effects were more pronounced among those over 65 years old, the Cambridge, Massachusetts-based biotech added, highlighting the vaccine's benefits for an age group more prone to COVID-19.


Similar to Moderna’s (MRNA) FDA approved vaccines, injection site pain, headache, and fatigue were some of the commonest side effects found in NextCOVE. The international trial enrolled more than 11,000 subjects aged 12 years and older in the U.S., U.K., and Canada.

There's been quite a few studies done in different countries tries all showing a statistically significant increase in cancers after the pandemic, that coincide with the large scale use of the mrna covid vaccines. Japan was the most recent one.

Only One Harm From mRNA COVID Vaccines, Report Determines

Independent reviewers confirmed a causal relationship between the first mRNA COVID-19 vaccines and myocarditis, and also determined that, more broadly, intramuscular shots can cause a series of shoulder injuries.

At the same time, the National Academies of Sciences, Engineering, and Medicine (NASEM) committee rejected a causal relationship between the Pfizer-BioNTech BNT162b2 and Moderna mRNA-1273 (Comirnaty and Spikevax, respectively) mRNA COVID vaccines and female infertility, Guillain-Barré syndrome, Bell's palsy, thrombosis with thrombocytopenia syndrome (TTS), and myocardial infarction.


The evidence was also deemed enough to reject a causal link between the Pfizer vaccine and ischemic stroke. There was insufficient data to say the same for the Moderna one, however, according to a reportopens in a new tab or window commissioned by the Health Resources and Services Administration (HRSA), a subagency of the HHS.

"Despite a large body of evidence from extraordinary efforts by investigators around the world, our committee found that in many cases, if not most, evidence was insufficient to accept or reject causality for a particular potential harm from a specific COVID-19 vaccine," said committee vice chair Anne Bass, MD, of Weill Cornell Medicine, Hospital for Special Surgery, and New York-Presbyterian Hospital, New York City, in a press releaseopens in a new tab or window.

"In other cases, we did find sufficient evidence to favor rejection, favor acceptance, or establish causality. It is important to note, however, that identifying a harm does not mean that it occurs frequently," she cautioned. "Harms associated with vaccines are rare."

Following a meeting of an advisory panel this week, the World Health Organization (WHO) recommended drugmakers to update their COVID-19 vaccine formulations to reflect the currently dominant SARS-CoV-2 variant, JN.1.

The decision came on Friday after the agency's technical experts on COVID-19 vaccines met early last week to determine the response of the latest crop of COVID shots to the fast-evolving SARS-CoV-2 virus.

According to data from the Centers for Disease Control and Prevention (CDC), Omicron JN.1 became the dominant COVID strain in the U.S. last December, days after the WHO classified it a "variant of interest" despite its low public health impact.

The currently approved COVID vaccines developed by Pfizer (NYSE:PFE), BioNTech (NASDAQ:BNTX), Moderna (NASDAQ:MRNA), and Novavax (NASDAQ:NVAX) are designed to target the XBB.1.5 Omicron subvariant.

The WHO said that the XBB.1.5-targeting vaccines would lose efficacy as the virus continues to evolve from JN.1.

"Given the displacement of XBB lineage variants by JN.1-derived variants, it is likely that, in the near term, circulating SARS-CoV-2 variants will be derived from JN.1," the agency noted.

While XBB.1.5-targeting vaccines offer limited protection against JN.1, "it is expected that the ability for XBB.1.5 vaccination to protect against symptomatic disease may be less robust as SARS-CoV-2 evolution continues from JN.1," the WHO said.
