Transfer Portal - How Does This Work?

Just read today that Joe will be entering the portal. How are these decisions made? Do the coaches tell them they will not see playing time next year so they best be getting out? Do the players figure this out on their own? Do the coaches rescind the scholarships? How does this all go down? Inquiring minds want to know.
